How to Create an Individual Registration Form - Step 3
In this tutorial, we're going to cover how you create an Individual registration form. Individual registration forms are used for participant membership, individual events such as clinics, camps, referee registration, etc. Step 3 is where policy information is entered on the form and where you can select the type of consent required in order to proceed with the registration.
- Log in to the Control Panel
- Click "Individual Registration Forms"
- Enter a name for your form (either the event name or type of registration)
- Select the "Form Type" (Player, coach, volunteer, registrant, referee)
- Click "Create Registration Form"
- Complete Steps 1 and 2 and proceed to Step 3
Instructions can be added above the Policies by clicking the link to "Click here to insert additional instructions at the top of Step 3."
Enter a waiver. Volleyball Canada has provided two separate waivers for participants of various ages. This waiver is designed to be used by Volleyball Canada, all PTAs and Clubs. The waiver is available at the following link and can be included as a link in the Waiver section of the policy page.
Waiver for those 18 years of age or older:

To create a link to a waiver, include some text such as "Please click here to view or download the waiver." Highlight the text and click the link icon. Enter the URL of the waiver and click ok to create the link.
Enter a privacy policy (or link to an existing policy)
Enter a refund policy
Enter the organization's contact information including a physical address
Select a consent option
- If no consent is required, select No from the dropdown list
- If consent is required in the form of a check box, select Yes - Checkbox from the dropdown list
- If consent is required in the form of text input, select Yes - Name Input from the drop down list
- Enter the consent terms (typically "Click here to agree to the above policy information" for example)
Click "Save Policies" and select to preview your form if desired or, proceed to step 4.
